- How do I obtain a copy?
Simply complete the attached Video Sales Waiver. Click here. Be sure to include the date and time of the newscast your story appeared as well as your choice of format (VHS or DVD) . Be sure to sign the waiver, include a check (payable to WJAR-TV) in the amount of $30.00 (includes shipping and handling) and send to:

- Are First Birthday's available?
Absolutely! Now you can order lasting memories of your child's First Birthday. In fact, you can order copies for your entire family.
- Can I request a story or First Birthday that aired prior to 2003?
Unfortunately, we cannot honor requests that aired prior to 1/1/03, except for our Timelines programs that are available for sale. Also, copies of entire newscasts are not offered under this program. NBC 10 WJAR-TV reserves the right to refuse requests.
- Does NBC 10 take credit cards?
No, sorry. We cannot process credit cards at this time.
- My check was returned, what happened?
When a check is returned, we include a note explaining why; but here are a few things to look out for:
- The check was not signed.
- The check was written for the wrong amount. The cost is $30.00 per story request.
- The waiver/request was not signed.
- The waiver/request did not indicate the date, show, or story title.
- The story requested is not included in our program.
- Where do I find the waiver/request?
There are three ways to obtain a waiver request: Click here, print the form, and mail it with your check to:
